TERMS OF AGREEMENT Please fill out the following application truthfully and completely. Failure to answer all questions may disqualify your application. This process is designed to appropriately match you or your family with a dog that will suit your lifestyle. While a particular dog may have caught your eye, it may not be the right fit for your home. It’s A Dog’s Life is based out of foster homes and has taken the time to understand the individual needs and temperaments of the dogs in our care to ensure they are adopted into appropriate homes. TERMS OF AGREEMENT Please read the following terms and conditions carefully. IT’S A DOG’S LIFE TERMS OF AGREEMENT ADOPTION FEES Adopter's are required to pay an adoption donation of $300.00 for all dogs adopted from It’s A Dog’s Life. These fees are used to attempt to recoup veterinary fees from vaccinations, spaying/neutering, and any other necessary vetting. This donation covers the puppy/dog’s first set of vaccinations and the spay/neuter and deworming. The adopter is responsible for the 2nd set of shots. ADOPTION TRIALS Dogs adopted from It’s A Dog’s Life will go to adoptive homes for a maximum two week trial period. If during this time the dog on trial will not be adopted by the home, it must be returned to It’s A Dog’s Life and a refund cheque will be provided. If after two weeks from the adoption being finalized the dog is returned to It’s A Dog’s Life the adoption donation will not be refunded. If at any time the adoptive home does not wish to keep the dog, during the trial period or after adoption has been finalized, it must be returned to It’s A Dog’s Life. If the dog has been placed, sold, given away, gifted, or adopted to another person or relinquished to or ownership has been transferred to an animal control, animal pound, humane society, shelter or rescue, It’s A Dog’s Life reserves the right to retrieve the dog. Unless for extreme health issues, no dog adopted from It’s A Dog’s Life may be euthanized or re-homed without written consent from this rescue. It’s A Dog’s Life reserves the right to repossess any adopted dog if the rescue finds reason(s) to suspect the dog is being mistreated, or if information on this form is found to be untruthful. The adopter(s) agrees to allow It’s A Dog’s Life to periodically check to confirm the dog's acceptance of the new environment and family (physically, mentally or otherwise) to ensure compliance with all Terms Of Agreement conditions and to verify that the animal has been spayed/neutered in accordance with the terms of this contract. PLEASE NOTE: WE WILL ONLY PAY FOR THE COST OF THE SPAY/NEUTER. ANY PRESURGERY APPOINTMENTS ARE THE RESPONSIBILITY OF THE OWNER AS IS ANY BLOOD WORK. This may include a home visit. It’s A Dog’s Life retains the right to visit and examine the animal at any time. It’s A Dog’s Life also retains the right to reclaim an animal when warranted because of neglect, abuse, inadequate home or veterinary care or breach of contract. SPAYING/NEUTERING All dogs adopted from It’s A Dog’s Life are required to be spayed or neutered. If adopted out prior to being spayed or neutered, the procedure must be performed: - within 15 days of adoption if the dog is over 6 months of age Female dogs going through heat cycles will be the only exception. They must be spayed immediately following the heat cycle and be kept safe from all male dogs. If It’s A Dog’s Life finds a dog has not been spayed or neutered within the timeframes stated on these forms, the rescue reserves the right to repossess the dog at any time. Adoption fees will not be returned. LEGAL OBLIGATIONS It’s A Dog’s Life cannot be held responsible for any damages that occur in the adoptive family's home, and is not liable for direct, indirect, incidental, special, or consequential damages arising out of, or relating to dogs adopted through It’s A Dog’s Life, whether framed as a breach of warrant, upon any legal theory, in tort, contract, failure of essential purpose, or otherwise. Adoptive families are obligated to report any concerns regarding their adopted dog to the rescue. All dogs adopted from It’s A Dog’s Life within the city of Kenora must be licensed with the City of Kenora Animal Services. PRIVACY POLICY Any information obtained through this and other forms on our website are kept on record with It’s A Dog’s Life. This information will not be shared with third parties, used in any solicitation, and will not be provided to any organization outside of It’s A Dog’s Life. In submitting this application, you agree to the terms of agreement as stated above. Until an adoption has been approved by an It’s A Dog’s Life director, all dogs remain the property of It’s A Dog’s Life.
